How to Request a Leave of Absence
- To request a Leave of Absence, go to My Info > My Time > Leave > Request.
- You may select the ellipsis, then Do I Qualify for FMLA? to complete a questionnaire to help you determine if you qualify for FMLA leave.
- Questions will appear in your To-Do items, which can be accessed via the Bell Icon at the top right of the screen.
Please note: Even if you do not qualify for FMLA, you may qualify for other types of leave based on your state’s regulations or company policies. Consult your HR department to discuss your options.
- Return to My Info > My Time > Leave > Request to complete the request form.
- Once complete, select Submit
- You will be notified via email once your request has been Approved or Rejected, and you may be asked to complete additional paperwork relating to your Leave of Absence.
Troubleshooting
Issue: I do not see the path My Info > My Time > Leave > Request.
Solution: Please see your HR Representative about requesting a Leave of Absence.
