The UKG Employee guide explains how to update withholding forms via Employee Self Service (ESS). Employees navigate to My Info My Pay Forms Withholding, click Add New, select the desired federal or state form, complete it, and save. The form shows as "New" until processed by HR or Payroll. To delete an unprocessed form, employees can use the trashcan icon or the actions menu under the same path.

1. How to Add a New withholding form
Employee logs in via ESS (Employee Self Service) and selects desired withholding form.
- Navigate to My Info > My Pay > Forms > Withholding
Click on the Add New button
Select the federal or state form you wish to update and click + Add button to the right of the form name/code.
Review instructions and proceed to complete the form. Required fields are notated with a red asterisk.
Once you have completed the entire form, click Review. Update any missing or incorrect fields, as needed.
- Once you have reviewed the form, click Sign & Submit.
- Apply your signature (or enter your password, depending on your company's system policies) and click I Agree.
The submitted form will show as Status: Employee Completed until processed by the Human Resource or Payroll Administrator.
2. How to delete an unprocessed form
If you wish to delete an unprocessed form, locate the form under My Pay > Forms > Withholding
